Fault Tree Analysis

A diagram that traces which combinations of events could cause a failure.

What is Fault Tree Analysis?

A diagram that traces which combinations of events could cause a failure.

What you learn

Know whether a failure needs both causes or just one.

When it helps

a serious failure may need several events to happen together and you must check which safeguards prevent it.

Use it at work

  1. Write the failure at the top.
  2. Below it, draw the events that could cause it.
  3. Mark whether they must happen together (AND) or alone (OR).
  4. Protect every cause that can trigger the failure alone.

Your output

For one serious failure, draw what must happen together and what can cause it alone. Protect the single points of failure.

Watch for

Treating every cause as equally risky, and removing safeguards that look redundant.
